- DS-82 Eligibility: Must have your passport in possession, undamaged, issued at age 16+ within the last 15 years, and in your current name.
- Check Writing: Must be written exactly to "U.S. Department of State" and signed. Insufficient payment will trigger rejection.
- Photo Standards: Must be 2" x 2" with white/off-white background, no glasses, printed on high-quality photo paper.
